Yes. We accept Medicare and PacificSource Medicare.
Yes. We accept all motor vehicle accident and personal injury (PIP) claims. An Auto Accident Form is available to fill out before your first visit.
Yes. We accept workers' compensation claims for on-the-job injuries.
No referral is needed to schedule a visit at Slater Chiropractic. Some insurance plans have their own requirements, so call us and we can help you check.
Bring your insurance card and the New Patient Health History Form (required). If they apply to you, also bring the Auto Accident Form or School Sports Exam Form. Call us and we will email the forms or have them ready at the front desk.
We review your health history and what brought you in, perform a thorough exam (with X-rays if needed), and then Dr. Bergman or Dr. Willow walks you through a treatment plan tailored to your goals.
Monday through Thursday 8:00am–5:30pm and Friday 8:00am–1:00pm. We are closed Saturday and Sunday.
